Maintenance Opioid Agonists Protect Against HCV in Injection Drug Users

Abstract

Importance Injection drug use is the primary mode of transmission for hepatitis C virus (HCV) infection. Prior studies suggest opioid agonist therapy may reduce the incidence of HCV infection among injection drug users; however, little is known about the effects of this therapy in younger users.

Objective To evaluate whether opioid agonist therapy was associated with a lower incidence of HCV infection in a cohort of young adult injection drug users.

Design, Setting, and Participants Observational cohort study conducted from January 3, 2000, through August 21, 2013, with quarterly interviews and blood sampling. We recruited young adult (younger than 30 years) injection drug users who were negative for anti-HCV antibody and/or HCV RNA.

Exposures Substance use treatment within the past 3 months, including non–opioid agonist forms of treatment, opioid agonist (methadone hydrochloride or buprenorphine hydrochloride) detoxification or maintenance therapy, or no treatment.

Main Outcomes and Measures Incident HCV infection documented with a new positive result for HCV RNA and/or HCV antibodies. Cumulative incidence rates (95% CI) of HCV infection were calculated assuming a Poisson distribution. Cox proportional hazards regression models were fit adjusting for age, sex, race, years of injection drug use, homelessness, and incarceration.

Results Baseline characteristics of the sample (n = 552) included median age of 23 (interquartile range, 20-26) years; 31.9% female; 73.1% white; 39.7% who did not graduate from high school; and 69.2% who were homeless. During the observation period of 680 person-years, 171 incident cases of HCV infection occurred (incidence rate, 25.1 [95% CI, 21.6-29.2] per 100 person-years). The rate ratio was significantly lower for participants who reported recent maintenance opioid agonist therapy (0.31 [95% CI, 0.14-0.65]; P = .001) but not for those who reported recent non–opioid agonist forms of treatment (0.63 [95% CI, 0.37-1.08]; P = .09) or opioid agonist detoxification (1.45 [95% CI, 0.80-2.69]; P = .23). After adjustment for other covariates, maintenance opioid agonist therapy was associated with lower relative hazards for acquiring HCV infection over time (adjusted hazard ratio, 0.39 [95% CI, 0.18-0.87]; P = .02).

Conclusions and Relevance In this cohort of young adult injection drug users, recent maintenance opioid agonist therapy was associated with a lower incidence of HCV infection. Maintenance treatment with methadone or buprenorphine for opioid use disorders may be an important strategy to prevent the spread of HCV infection among young injection drug users.

from Jules: with the advent & revolution of new HCV oral & interferon-free therapy it will be important to provide education to at-risk patients about the risks for-re-infection with HCV. It is important to provide treatment to IDUs as persons but also for society. All too often now treatment for IDUs is withheld for a number of reasons including because IDUs are at risk for continuing risky behavior, sharing unclean needles & getting re-infected. Instead treatment for IDUs should be viewed as important for the patient, for prevention, ad for society & treatment should be viewed as an opportunity to provide education about preventing re-infection, why & how the patient should not be re-infected. Often this means addressing the patient's risky behavior, which could be sharing used/unclean syringes for the IDU or continued risky sexual & drug behavior for MSM. In recent studies in NYC & in London re-infection was cited among MSM due to continued unsafe drug & sex behavior, in fact re-infection was reported to occur 2-3 times after successful treatment for some individuals. In recent studies re-infection among IDUs has been found often. With treatment & cure of HCV comes a responsibility that these at-risk individuals, IDUs or a history of IDU, are educated not to be re-infected. Resources are scarce and should not be wasted particularly in the developing & undeveloped world but also in Europe, the USA & the Western world, its possible that retreatment may be denied by government or payers.

Author's CONCLUSIONS: This study highlights the need for broadly implemented HCV prevention interventions for all IDUs seeking addiction treatment, and suggests such interventions might particularly decrease transmission behaviors by those aware of their HCV infection and prevent HCV infection in those HCV-negative/unaware. Research which prospectively studies the effect of HCV testing and notification on risk behavior could help further clarify the association between HCV awareness and risk behaviors. Interventions that could improve services for IDUs include those that explicitly and repeatedly educate IDUs about safer injection practices and the treatability of HCV, and those that integrate HCV testing and treatment with addiction treatment services. As HCV screening and treatment options advance, community based treatment programs have a greater opportunity to play a central role in reducing HCV transmission and engaging HCV-infected IDUs in treatment.

Abstract

Background

Injection drug use (IDU) is a primary vector for blood-borne infections. Awareness of Hepatitis C virus (HCV) infection status may affect risky injection behaviors. This study determines the prevalence of risky injection practices and examines associations between awareness of positive HCV status and risky injection behaviors.

Methods

We surveyed individuals seeking treatment for substance use at 12 community treatment programs as part of a national HIV screening trial conducted within the National Drug Abuse Treatment Clinical Trials Network. Participants reported socio-demographic characteristics, substance use, risk behaviors, and HCV status. We used multivariable logistic regression to test associations between participant characteristics and syringe/needle sharing.

Results

The 1281 participants included 244 (19.0%) individuals who reported injecting drugs in the past 6 months and 37.7% of IDUs reported being HCV positive. During the six months preceding baseline assessment, the majority of IDUs reported obtaining sterile syringes from pharmacies (51.6%) or syringe exchange programs (25.0%), but fewer than half of IDUs always used a sterile syringe (46.9%). More than one-third (38.5%) shared syringe/needles with another injector in the past 6 months. Awareness of positive HCV vs. negative/unknown status was associated with increased recent syringe/needle sharing (aOR 2.37, 95% CI 1.15, 4.88) in multivariable analysis.

Conclusions

Risky injection behaviors remain prevalent and awareness of HCV infection was associated with increased risky injection behaviors. New approaches are needed to broadly implement HCV prevention interventions for IDUs seeking addiction treatment.

1. Introduction

Injection drug use (IDU) is the primary driver for Hepatitis C virus (HCV) transmission, accounting for the majority of chronic HCV infections in the U.S. (Alter, 1999 and Armstrong et al., 2006). Across multiple U.S. studies, 35-65% of current IDUs report risky injection behaviors such as syringe/needle sharing (Bailey et al., 2007, Booth et al., 1998, Centers for Disease Control and Prevention, 2009 and Golub et al., 2007).

Knowledge of harboring a transmissible infection such as HCV may influence risky behaviors. Prior studies of the effects of HCV-infection awareness on risky behaviors demonstrate mixed results. In a serosurvey of out-of-treatment IDUs, those who reported awareness of HCV-infection engaged in fewer risky behaviors compared with those who were unaware (Kwiatkowski, Fortuin Corsi, & Booth, 2002). HCV-aware IDUs may also "sero-sort," or preferentially engage in risky injection behaviors with others they know to be similarly HCV-infected (Burt, Thiede, & Hagan, 2009). Other studies, however, suggest that HCV awareness is insufficient to change injection risk behaviors (Norden et al., 2009). Little is known about the influence of HCV awareness on IDUs engaged in substance use treatment-information that might improve HCV prevention services in addiction treatment settings.

The purpose of this analysis was to 1) assess the prevalence and correlates of drug use practices among patients presenting for addiction treatment and 2) compare risky behaviors in those reporting HCV-infection with those who reported negative or unknown HCV status.

2. Methods

2.1. Design and setting

The primary study was a National Drug Abuse Treatment Clinical Trials Network (CTN) trial comparing the effectiveness of strategies to increase HIV testing (Metsch et al., in press). Between January and May 2009, the trial randomized 1281 individuals receiving addiction treatment at 12 geographically diverse, community-based addiction treatment programs. After providing informed consent, participants completed an audio computer assisted self interview recording substance use behaviors.

2.2. Participants

Participants receiving addiction treatment were eligible for enrollment if they were 1) ³ 18 years old, 2) reported unknown or negative HIV status, and 3) had not been tested and received results for HIV within the last 12 months. The current analysis was restricted to the 244 participants who reported IDU in the six months preceding the study baseline assessment.

2.3. Measures

Participants were asked about injection risk behaviors over the prior six months using items from Project Inspire (Purcell et al., 2004) and the NIDA Risk Behavior Assessment survey (Needle et al., 1995) including source of syringes, needle cleaning practices, how they cleaned their needles, and recent syringe/needle sharing (the main dependent variable). Participants reported injection and non-injection drug use and drinking alcohol to intoxication in the past 6 months (Colfax et al., 2004).

The independent variable was self-reported HCV infection awareness. Patients were asked, "Have you ever been diagnosed with hepatitis C (yes, no, don't know)?" Because there was no difference in syringe/needle sharing between participants who reported they were HCV-negative and those who did not know their HCV status, we dichotomized this variable as HCV-positive vs.

HCV-negative/unknown. Covariates included age, gender, race/ethnicity, employment, education, court-mandated treatment, opioid replacement treatment, and whether or not the patient had been jailed in the last 6 months.

2.4. Analysis

Descriptive statistics characterized participant socio-demographics, and substance use behaviors. We assessed bivariate and multivariable associations between participant characteristics and any syringe/needle sharing using logistic regression. Variables were included in the multivariable logistic regression model if associated with syringe/needle sharing in univariate analyses (p < .20), or on the basis of a priori hypotheses. Potential interactions were assessed.

3. Results

3.1. Participant characteristics

Of 244 recent IDUs, 60.7% were men, 66.0% white, 14.3% Hispanic, and 10.2% Black race/ethnicity, with a mean age of 39.3 (SD = 11.0) years. Twenty percent were employed, 36.2% had attained at least some college education, 30.3% had been recently incarcerated, 20.1% were receiving court-mandated treatment and 46.7% opioid replacement therapy. Ninety-two IDU (37.7%) reported being positive for HCV, 55 (22.5%) HCV-negative, and 97 (39.8%) unknown HCV status. Compared with those who were HCV negative/unaware, HCV positive IDUs were older (45.3 vs. 35.6 years, p < .001), more likely to be women (52.2% vs. 31.6%, p = .001) or enrolled in opioid replacement programs (68.5% vs. 33.6%, p < .001) and less likely to be recently incarcerated (21.7% vs. 35.5%, p = .023).

The most commonly used substances were injected opioids (71.17%), drinking alcohol to intoxication (70.9%), non-injection opioids (66.4%), marijuana (48.8%), crack cocaine (45.1%), and cocaine (30.7%). The majority of IDUs (81.1%) injected more than one substance at a time. HCV positive IDUs were less likely to drink alcohol to intoxication (57.6% vs. 78.9%, p < .001) but more likely to inject heroin (68.5% vs. 55.3%, p = .041) compared with HCV negative/unaware.

3.2. Injection risk behaviors

More than one third (38.5%) of IDUs reported syringe/needles sharing in the past 6 months (Table 1). IDUs obtained needles mostly from pharmacies, syringe exchange programs, and diabetic supplies. Less than half always used a clean needle. Among IDUs who cleaned their needles, cleaning with bleach was the most common method, but many used more ineffective sterilization methods including soap and water. More HCV-positive IDUs reported recent syringe/needle sharing compared with those with HCV negative/unknown status (44.6% vs. 38.5%, p = .131), though this was not statistically significant. There was no difference in recent syringe/needle sharing between those who reported being HCV negative vs. unknown status (36.4% vs. 34.0%, p = .771). HCV positive IDUs more frequently obtained needles from a syringe exchange program and used bleach if they cleaned needles.

In adjusted analysis (Table 2), recent syringe/needle sharing was more likely among those who reported they were HCV-positive compared with those who were HCV negative/unaware (aOR 2.37 [95% CI 1.15, 4.88]), and among IDUs obtaining needles from the street, using any opioids, marijuana, or injected crack cocaine; sharing was less likely among males and participants with some college education.

4. Discussion

Risky injection practices persist among IDUs, with rates in the current study consistent with those of other recent studies (Booth et al., 2011 and Centers for Disease Control and Prevention, 2009) and may partially explain persistently high HCV incidence among IDUs (Mehta et al., 2011). In a survey of IDUs in 23 U.S. cities from 2005 to 2006, 31.8% of IDUs reported sharing needles (Centers for Disease Control & Prevention, 2009). Among IDUs enrolling in a behavioral intervention trial (2-session HIV/HCV counseling vs. therapeutic alliance vs. treatment as usual) at residential detoxification centers from 2004 to 2006, 61% reported sharing needles, works, or drug solution (Booth et al., 2011). More widespread adoption of interventions demonstrated to reduce risky injection practices, and development of new, more effective interventions, are urgently needed for patients enrolling in community-based treatment programs.

The majority of IDUs in the present study obtained needles from safe sources, including pharmacies and syringe exchange programs, as corroborated in surveys of IDUs in other U.S. cities with policies that increase availability of sterile needles and syringes (Golub et al., 2005 and Khoshnood et al., 2000) - policies that decrease HIV transmission and likely decrease HCV transmission, as well (Des Jarlais et al., 1996 and Des Jarlais et al., 2000). Despite this, fewer than half of IDUs reported always using a clean needle or consistently cleaning needles, indicating that needle re-use and lack of needle cleaning are common. Even among the minority of IDUs reporting consistent needle cleaning, sterilization techniques other than use of bleach were frequently employed, suggesting that renewed efforts are needed to promote harm reduction techniques among IDUs. Interventions that promote needle cleaning such as peer-based (Hawkins et al., 1999 and Rietmeijer et al., 1996), pharmacy-based (Romanelli, Smith, & Pomeroy, 2000), and provider-based (Carlson, Wang, Siegal, & Falck, 1998) interventions, continue to be relevant for IDUs engaged in community-based treatment. At the same time, renewed efforts to increase availability of clean syringe/needles are urgently needed to decrease HCV transmission.

Participants who reported they were HCV positive differed in several important behaviors compared with their counterparts. HCV positive IDUs more frequently exhibited harm reduction behaviors such as obtaining needles from a syringe exchange program, cleaning needles with bleach, and avoiding drinking alcohol to intoxication, suggesting that awareness of HCV status may confer increased adoption of some protective behaviors. HCV positive IDUs, however, were also more likely to inject heroin and, in multivariable analysis, to share needles. The observed association between HCV-awareness and increased needle/syringe sharing may reflect a complex cluster of characteristics among HCV-aware IDUs in this cross-sectional study. Indeed, a recent synthesis of qualitative studies of HCV risk among IDUs identified risk ubiquity as a common theme, supporting a perception of HCV as "a risk accepted rather than avoided" (Rhodes, Singer, Bourgois, Friedman, & Strathdee, 2005). This is consistent with findings from a multicenter study of Swedish IDUs, in which 74% of those HCV-aware shared needles compared with 68% of those with unknown status (Norden et al., 2009).

The current study confirms the importance of certain demographic and drug use characteristics previously associated with syringe/needle sharing including younger age, female gender, lower educational attainment, and use of opiates, and crack cocaine. While greater addiction severity is associated with riskier injection behaviors, the current study is among the first to identify an association between marijuana use and risky injection behaviors. Marijuana use in IDUs may be a marker of risk-taking personality or chronically decreased motivation to protect oneself, as hypothesized to explain similar findings in a study of Russian IDUs (Walley et al., 2008). Further research is required to assess the nature of this association.

This study has limitations. First, our study population was recruited from individuals seeking or actively engaged in treatment in community-based treatment programs. Findings may not be generalizable to IDUs in other settings. Second, HCV status was assessed by self-report and likely underestimates the actual prevalence of HCV. However, one's belief about one's HCV status is conceptually more closely related to injection behaviors than biologically confirmed HCV status. Third, we were unable to assess sero-sorting in the current study, so increased sharing among HCV may have been with other known HCV-positive IDU, as was observed in one prior study (Burt et al., 2009). Finally, the current study's cross-sectional design limits our ability to infer causality. The observed multivariable association between knowledge of HCV status and syringe/needle sharing may reflect overall greater drug use severity among those who become HCV-infected rather than a causal pathway toward increased risky behaviors. Regardless, the association highlights the role of HCV awareness as a marker for IDUs in particular need of harm reduction interventions.

5. Conclusions

This study highlights the need for broadly implemented HCV prevention interventions for all IDUs seeking addiction treatment, and suggests such interventions might particularly decrease transmission behaviors by those aware of their HCV infection and prevent HCV infection in those HCV-negative/unaware. Research which prospectively studies the effect of HCV testing and notification on risk behavior could help further clarify the association between HCV awareness and risk behaviors. Abstract

Background and Aim

Chronic hepatitis C virus (HCV) infection is one of the leading causes of cirrhosis and hepatocellular carcinoma worldwide. It is highly prevalent among injection drug users (IDUs) but is often undiagnosed because they represent an underprivileged group that faces multiple barriers to medical care. Here we report the results of the New Life New Liver Project, which provides targeted HCV screening and education for ex-IDUs in the community.

Methods

Patients were recruited through the social worker networks and referrals by fellow ex-IDUs, and rapid diagnosis was based on point-of-care anti-HCV testing at rehabilitation centers.

Results

From 2009 to 2012, we served 234 subjects. 130 (56%) subjects were anti-HCV positive. The number needed to screen to detect 1 patient with positive anti-HCV was 1.8 (95% confidence interval, 1.6-2.0). However, only 69 (53%) HCV patients attended subsequent follow-up at regional hospitals, and 26 (20%) received antiviral therapy. Patients who attended follow-up were older, had higher education level and more active disease as evidenced by higher alanine aminotransferase, HCV RNA and liver stiffness measurement by transient elastography.

Conclusions

Targeted screening in ex-IDUs is effective in identifying patients with HCV infection in the community. Improvement in the referral system and introduction of interferon-free regimens are needed to increase treatment uptake.

Multiple Tactics Needed to Combat Hepatitis C in Young Drug Users

With hepatitis C infections on the rise among young injection drug users, researchers have recommended a range of promising prevention tactics.

The number of young adults infected with the hepatitis C virus (HCV) is on the rise, and researchers estimate that more than 31,000 young injection drug users could become infected each year in the United States. To help reduce the rate of HCV infection in young drug users, the authors of a new review suggest 6 strategies to prevent the spread of the virus.

The recommendations, published in an August 15, 2013, supplement to Clinical Infectious Diseases, are based primarily on evidence from the large, ongoing “U Find Out” (UFO) study. The study, which began in 2000, enrolls HCV-negative injection drug users younger than 30 in San Francisco, California, and keeps track of the rate of new infections. This data, coupled with results of additional studies, has helped researchers identify a range of promising HCV prevention tactics.

The first suggestion focuses on the reduction of shared drug equipment. A meta-analysis of 21 studies found that the risk of contracting HCV from shared use of drug preparation containers, filters, and rinse water was similar to the risk of infection associated with sharing syringes. If drug preparation equipment sharing were eliminated, the results indicated, a large portion of HCV infections could be prevented. The authors of the current review note that education, training, and increasing the availability of single-use supplies are necessary to prevent new HCV infections.

Although little research has been done to evaluate the efficacy of safety education in injectors, the review authors next suggest that testing and counseling populations at risk for HCV may reduce risky behavior as well as the spread of the virus. In the UFO study, researchers found that drug injectors reduced their alcohol intake and used less non-injection drugs within 6 months of receiving HCV-positive test results, indicating that testing may reduce risky behaviors. The authors are optimistic that the availability of rapid HCV point-of-care tests, recently approved by the FDA, may increase rates of HCV testing, diagnosing, and treatment.

The third tactic recommended by the authors is to develop intervention strategies at a relationship level, as drug use is often a social activity. The UFO study indicates that injectors in sexual relationships with other injectors are more likely to share needles with their partners and, therefore, are at heightened risk of HCV infection. Given the results of a recent study that showed that couple-based testing and counseling effectively reduced risky behaviors in injectors, the authors note that researchers should assess the impact of these interventions on HCV outcomes.

The reviewers next recommend an increase in injection cessation interventions. Evidence from the UFO study suggests that even temporary injection cessation helps prevent HCV infections. The authors note that encouraging injectors to quit, even after multiple failed attempts, may prevent the spread of HCV infections and increase the number of users who quit permanently.

The fifth strategy, based on mathematical models, is to increase needle distribution, expand HCV treatment, and continue to research and develop vaccines against HCV. Researchers modeled the effects of syringe availability on HIV and HCV prevalence among drug injectors in Australia and estimated that increasing the number of needles distributed each year in the United States from 30 million to 60 million could potentially cut the number of new HCV infections in half.

Finally, the authors suggest the adoption of programs combining multiple prevention strategies. The results of a study conducted in the United Kingdom suggested that high coverage of opioid substitution treatment combined with syringe distribution programs could reduce the risk of new HCV infections by up to 80%. Combined prevention methods were also shown to be significantly more effective in reducing HCV rates than a single method in 2 separate studies.

Although the authors have identified strategies they believe will help reduce the spread of HCV, they note that political backing and additional resources will be needed to implement these tactics and to overcome the many obstacles to reducing the infection rate in young injectors.

Hepatitis C: 6 Prevention Strategies That Work

Hepatitis C virus (HCV) infection is on the rise, particularly among young white adults living in rural and suburban areas who abuse intravenous opiate drugs. A review article published in a special supplement to Clinical Infectious Diseases, coinciding with World Hepatitis Week, discusses 6 ways to combat the spread of HCV in this group.

HCV incidence is increasing among those aged 15 to 30 years who inject drugs, according to the US Department of Health and Human Services. HCV is 10 times as infectious as HIV and surpassed HIV in number of deaths in the United States in 2007. Nearly 4 million people in the United States have chronic HCV infection.

For 16 years, Kimberly Page, PhD, MPH, from the University of California, San Francisco, and colleagues have conducted the "U Find Out," or UFO, Study with injection-drug users. The program is modeled on clean syringe programs, but with greater emphasis on social issues that fuel drug abuse and on integrating multiple approaches to combat HCV infection. Data from the ongoing prospective study show that the interventions used can reduce the risk of seroconversion.

In addition, the investigators note that more than 31,000 young adults who inject drugs of abuse will become infected with HCV in the United States each year. "Although there is evidence that HCV incidence has declined in recent decades, if the number of young injectors increases, as the [Centers for Disease Control and Prevention] suggests is occurring in the United States, these gains could be lost. Ongoing and targeted surveillance efforts to enumerate the population at risk and assess the burden of infection are essential steps in targeting and implementing effective HCV prevention in this group," the authors write.

Therefore, they recommend 6 comprehensive measures to prevent the spread of hepatitis C:

  1. Reducing risk from shared ancillary drug preparation equipment, such as containers, rinse water, and filters. The researchers estimate that paying attention to the ancillaries as well as shared syringes could prevent at least 25% of seroconversions. The availability of single-use supplies could lower risk.

  2. Using a new rapid test at point of care that offers results in 20 minutes can detect infection before seroconversion and, combined with counseling, can help to stem transmission.

  3. Addressing social and relational contexts of injecting can encourage uninfected individuals to take precautions when injecting drugs with infected sex partners.

  4. Encouraging "taking a break" from injecting drugs because HCV cannot be transmitted without this behavior. In past studies, the investigators discovered that the more often drug abusers attempted to quit, the more likely they were to eventually succeed.

  5. Developing models to guide delivery of new prevention strategies, including already-available approaches such as increasing syringe availability and future strategies such as direct-acting antivirals that can be used prophylactically, as well as vaccines.

  6. Combining interventions in synergistic ways, such as needle exchange and methadone maintenance programs.

"With ongoing and expanding transmission of HCV, there is little doubt that there is a need to implement what is in the prevention 'toolbox,' as well as add to it," they write. "Expanding HCV prevention and reaching the new generation of young injectors will require dedicated advocacy, pragmatism, and persistence to enable access to all of these technologies."

(PROVIDENCE, R.I.) – A Miriam Hospital researcher has joined forces with international colleagues to call for new strategies to better manage and improve assessment and treatment for hepatitis C (HCV) infection in individuals who inject drugs.

Lynn E. Taylor, M.D., an HIV specialist focusing on HIV and viral hepatitis coinfection at The Miriam Hospital, was the only American physician invited to join the expert international panel that issued these first-of-its-kind recommendations. They were published online yesterday by the journal Clinical Infectious Diseases, just ahead of World Hepatitis Day on July 28.

The recommendations are part of a supplement entitled "Prevention and Management of Hepatitis C Virus Infection Among People Who Inject Drugs: Moving the Agenda Forward," developed in collaboration with the International Network on Hepatitis Care in Substance Users.

"In well-resourced parts of the world, most hepatitis C exists among people who currently inject drugs and those who have injected drugs in the past. However, treatment access and uptake among this population remains low – even though we increasingly have effective treatments for hepatitis C, which is a curable disease," said Taylor.

"Research supporting our recommendations – the first international set ever released for treating hepatitis C in people who inject drugs – demonstrates that treatment can be successful when barriers to care are addressed within a supportive environment," she added. "In fact, the burden of liver disease worldwide could be dramatically reduced by increasing treatment for hepatitis C infection among people who inject drugs, by preventing forward transmission."

An estimated five million people in the U.S. have chronic HCV infection, a liver disease that may result in long-term health problems, including liver scarring, liver failure and liver cancer. According to the Centers for Disease Control and Prevention, approximately 12,000 people die every year from HCV-related liver disease.

Until recently, HCV treatment guidelines excluded people who inject drugs, due to concerns about poor adherence, adverse events and re-infection. However, successful HCV treatment studies among this population have challenged this paradigm. The new international guidelines present evidence-based recommendations for treating HCV among individuals who inject drugs with appropriate evaluation and support.

Taylor is also lead author on a separate paper, appearing in the same supplement of Clinical Infectious Diseases, which focuses on the need for improved HCV care of another subset of the HCV-infected population: those who inject drugs and are also infected with HIV.

Chronic HCV infection has become a leading cause of non-AIDS related illness and death among individuals infected with HIV. Due to overlapping routes of transmission, dual infection is common: in the United States, 30 percent of HIV-infected people have chronic HCV, which is spread via contaminated blood, often through injection drug use. However, newer research suggests it may also be transmitted sexually among HIV-infected men who have sex with other men.

"HIV-infected individuals contending with injection drug use are the most likely to be affected by HCV, but the least likely to have access to treatment for HCV," said Taylor. "They should have equal and universal access to HIV/AIDS, HCV and addiction prevention, care and treatment."

She says essential but basic steps include improving prevention and screening for both infections and engaging co-infected individuals who inject drugs in HIV and HCV care early after diagnoses.

"The benefits of therapeutic advances in HCV will be limited for this group until barriers such as cost and access are overcome," she added. "Even with HCV cure rates approaching 100 percent with newer medications, effectiveness at population level will require expanding HCV therapy on large scale. These recommendations are an important step towards the goal of elimination of hepatitis C."

Taylor is also director of the HIV/Viral Hepatitis Program at The Miriam Hospital and an assistant professor of medicine at The Warren Alpert Medical School of Brown University.

Combining treatments for people who inject drugs is the first step towards eliminating hepatitis C

The burden of liver disease could be dramatically reduced by scaling up the combination of interventions for hepatitis C infection among people who inject drugs according to University of Bristol researchers. The findings, published today [24 July], form part of new global recommendations on treating the virus.

Around 150 million people globally are chronically infected with the hepatitis C virus (HCV)1 – a major cause of liver disease that can lead to serious complications such as liver failure or cancer, which are associated with considerable costs to the health care system. In developed countries the majority of transmissions and cases are among people who inject drugs2 – in the UK, this equates to around 90 per cent of hepatitis C infections.3

The study, published today in the journal Clinical Infectious Diseases by Dr Natasha Martin and colleagues at University of Bristol, the London School of Hygiene and Tropical Medicine and Health Protection Scotland, suggests that combining HCV antiviral treatment with opiate substitution therapy (OST) such as methadone and buprenorphine, and high-coverage needle and syringe programmes (HCNSP) is critical for achieving substantial reductions by up to 50 per cent over ten years.

Matthew Hickman, Professor in Public Health and Epidemiology at Bristol's School of Social and Community Medicine and co-author of the research, said: "High-coverage needle and syringe programmes (NSP) and opiate substitution therapy (OST) are the key primary interventions to treat hepatitis C among people who inject drugs; but HCV treatment is required if we want to achieve greater than 45 per cent reduction in HCV prevalence over a ten year period.

"Our model projections show that scaling up OST and high coverage NSP can reduce the number of HCV treatments required to achieve reductions in HCV – and emphasises the importance of a combination of interventions. Further research is needed to examine the cost effectiveness and affordability of scaling up HCV treatment."

The research supports the first set of global recommendations, published by the International Network on Hepatitis Care in Substance Users (INHSU), ever released for treating hepatitis C in people who inject drugs which has shown that treatment can be very successful when barriers are addressed within a supportive environment.

Philip Bruggmann, President of the INHSU, who have published the recommendations on treating the disease in substance users, said: "Reducing the significant burden of liver disease related to hepatitis C internationally will require improved HCV care in the population most affected: those people who currently inject or formerly injected drugs. By providing appropriate care to this group, we can reduce the burden of hepatitis C-related liver disease in this vulnerable population and slow the spread of this global epidemic. These new recommendations serve as a first step in eliminating hepatitis C."

Hepatitis C is a blood-borne virus that predominantly infects the cells of the liver. This can result in inflammation and significant damage to the liver. It can also affect the liver's ability to perform its essential functions. Six Recommended Measures to Prevent Hepatitis C for Young Injection-Drug Users

UC San Francisco researchers are recommending a combination of six comprehensive measures to prevent the spread of hepatitis C, in an effort to address the estimated 31,000 young people who may be newly infected each year in the United States due to injection-drug use.

The measures, which stem from a 16-year UCSF research project with injection-drug users, known as the “U Find Out” or UFO Study, build upon the successes of clean syringe programs and similar efforts, while recommending greater focus on the social issues behind drug use and further integration of the multiple approaches to combating hepatitis C.

In February, the U.S. Department of Health and Human Services (HHS) noted the rising epidemic of hepatitis C virus among young people aged 15-30 who inject drugs, calling attention to an increasingly serious issue nationwide.

While data on hepatitis C are limited, the HHS estimates that as many as 3.9 million people in the United States are living with a chronic form of the disease, which the researchers said is at least 10 times more infectious than HIV. In 2007, the number of U.S. deaths associated with hepatitis C surpassed those from HIV for the first time.

“Based on our UFO Study here in San Francisco, we have accumulated data that identify key strategies that, when scaled up, could substantially reduce the rate of new hepatitis C infections among young people who inject drugs,” said the study’s lead investigator, Kimberly Page, PhD, MPH, a professor in the UCSF Department of Epidemiology and Biostatistics, and in UCSF Global Health Sciences.

The research team examined several data sources to arrive at the new estimate of 31,000 new cases per year and identified six areas where prevention efforts should be focused. Findings appear online July 24 in a special supplement titled, “Prevention and Management of Hepatitis C Virus among People Who Inject Drugs: Moving the Agenda Forward,” in the journal Clinical Infectious Diseases. The publication coincides with World Hepatitis Week, July 21 to 28.

Beyond Syringe Exchange Programs

First, while syringe-exchange programs have long been recognized as an absolutely essential element in disease prevention, giving injectors access to clean needles and syringes, the UFO Study team discovered that up to 40 percent of infections occurred from exposures to shared drug preparation containers, filters and rinse water.

“The hepatitis C virus lives a long time on surfaces and can easily contaminate various types of injecting equipment, so while expanding needle exchanges throughout the country is essential, one of our critical recommendations is that existing and newly established exchanges provide clean ancillary equipment along with needles and syringes,” said Page.

The "U Find Out" Study, which began in September 1997, is a constellation of research studies and projects focusing primarily on viral hepatitis, particularly hepatitis C virus, among young injection drug users in San Francisco.

Additional strategies identified in the paper include hepatitis C virus screening, testing and counseling; targeting interventions to address the social and relational contexts of injecting; injection cessation interventions to reduce risks of exposure; development of models to guide roll out of new hepatitis C treatments and vaccines; and the implementation of robust, scaled interventions in combination to synergize the effectiveness of individual interventions.

“A new rapid test for hepatitis C has become available that can be delivered at the point of care with results in 20 minutes, so making that widely available can significantly improve screening, testing and counseling,” said Page.  “Also, our research identified the value of ‘taking breaks’ for both reducing the risks of exposure to the virus and helping end injection drug use.”

The UFO Study looked at the behaviors associated with attempts by injectors to complete various injection cessation programs, such as 12-step or opiate substitution programs. While in these programs, exposure risks disappeared.

However, while injectors frequently fail cessation programs, the UFO Study showed that the more injectors try to quit, or “take breaks” from drugs, the more likely they eventually are to succeed. That new data could inform how these and other treatment programs handle relapses in the future.

“Often these programs are very unforgiving and relapses can lead to permanent bans,” Page said. “This is likely not the best tactic for reducing disease exposure risks and also may not be optimal for helping injectors end injection drug use.”

There was a significant reduction in hepatitis C incidence in Australia among injection drug users who attended needle and syringe programs, according to data published in the American Journal of Public Health.

“The decline in new cases of hepatitis C among people who inject drugs coincided with the expansion of programs like opioid substitution therapy and needle and syringe programs, which aim to minimize the spread of bloodborne viruses,” Jenny Iversen, a PhD candidate at the Kirby Institute at the University of New South Wales in Sydney, said in a press release. “We also found that fewer young people are starting to inject drugs and that the types of drugs people are injecting have changed, which may have contributed to a decline in the number of people contracting the virus.”

Iversen and colleagues conducted a passive retrospective study that included a cohort of 724 injection drug users who tested negative for HCV antibodies. The participants had been part of the annual Australian Needle and Syringe Programs Survey and had completed the survey for consecutive years or with only a 1-year gap from 1995 to 2010.

Among the 724 participants, 180 had HCV seroconversions, for a pooled incidence density of 17 per 100 person-years (95% CI, 14.68-19.66). The incidence density varied each year, peaking at 30.8 per 100 person-years in 2003 (95% CI, 21.3-44.6), then declining to 4 per 100 person-years in 2009 (95% CI, 1.3-12.3). In a multivariable analysis, independent predictors of HCV seroconversion included imprisonment in the previous 12 months, daily or more frequent injection, residence in mainland eastern Australia and injection of cocaine, heroin or other drugs, compared with injection of methamphetamine.

“These results demonstrate the value of ongoing investment in Australia’s internationally recognized system for the surveillance of HIV and hepatitis C among people who inject drugs,” Lisa Maher, PhD, head of the viral hepatitis epidemiology and prevention program at Kirby Institute, said in a press release. “This is the first time in the world this method has been used to estimate hepatitis C incidence and the first report of incidence in a national sample of people who inject drugs. It allows us to assess the uptake and impact of prevention and treatment interventions in this group.”
